2. On a new white 8.5 × 11 inch sheet, hand print your full name, your mailing address and that code.
3. Upload a clear photo of the sheet with the form and submit it.

Each submission is worth 25 entries. One submission per person per day. Free entries are drawn on the same terms as entries earned by buying Seals.
